Yesterday, NIST announced the AI Agent Standards Initiative — a coordinated federal effort to establish standards for AI agent security, identity, and interoperability. This isn't a research paper or a framework. It's an active call for industry input with deadlines in the next few weeks.
If you're deploying AI agents in healthcare — or evaluating vendors who are — this matters.
What NIST Announced
The Center for AI Standards and Innovation (CAISI) launched the initiative with three pillars:- Industry-led development of agent standards and U.S. leadership in international standards bodies
- Community-led open source protocol development for agents (MCP is explicitly mentioned)
- Research on AI agent security and identity to enable trusted adoption
Two documents are open for public comment right now:

The NCCOE concept paper is particularly significant. It's not theoretical — it's scoping a demonstration project to show how existing identity standards (OAuth, OIDC, SPIFFE/SPIRE) can be applied to AI agents. They're asking for real-world use cases and implementation feedback.
Why This Matters Now
NIST's framing is telling. From the announcement:"AI agents can now work autonomously for hours, write and debug code, manage emails and calendars, and shop for goods... Absent confidence in the reliability of AI agents and interoperability among agents and digital resources, innovators may face a fragmented ecosystem and stunted adoption."
They're not worried about AI agents as a future possibility. They're worried about AI agents being deployed today without adequate security standards. The OpenClaw/Moltbot phenomenon — autonomous agents running for extended periods, accessing production systems — is explicitly what they're responding to.
For healthcare, the stakes are higher. An agent that "manages emails" in a general enterprise context becomes an agent that "accesses patient communications" in healthcare. An agent that "shops for goods" becomes an agent that "places orders in supply chain systems" with PHI implications.
The Identity Problem NIST is Trying to Solve
The NCCOE concept paper asks a series of questions that reveal the core challenge. I've grouped them by theme:Identification:
- How might agents be identified in an enterprise architecture?
- What metadata is essential for an AI agent's identity?
- Should agent identity metadata be ephemeral (task-dependent) or fixed?
Authentication:
- What constitutes strong authentication for an AI agent?
- How do we handle key management for agents? Issuance, update, revocation?
Authorization:
- How can zero-trust principles be applied to agent authorization?
- How do we establish "least privilege" for an agent, especially when its required actions might not be fully predictable?
- How might an agent convey the intent of its actions?
- How do we handle delegation of authority for "on behalf of" scenarios?
Auditing:
- How can we ensure that agents log their actions and intent in a tamper-proof and verifiable manner?
- How do we ensure non-repudiation for agent actions and binding back to human authorization?
Prompt Injection:
- What controls help prevent both direct and indirect prompt injections?
- After prompt injection occurs, what controls can minimize the impact?
These aren't abstract research questions. NIST is asking because they don't have complete answers, and they need industry input to build practical guidance.
How Multi-Layered AI Identity Maps to NIST's Questions
Over the past few months, I've been developing a framework called Multi-Layered AI Identity that addresses exactly these challenges. Here's how the four identity layers map to what NIST is asking:Agent Identity → NIST's Identification and Authentication Questions
NIST asks: "How might agents be identified? What constitutes strong authentication?"Agent Identity is checkpoint-based verification at instantiation:
- Agent authenticates with Identity Provider
- Receives dynamic, short-lived credentials
- Agent instance is validated against authorized configurations
This uses existing patterns (SPIFFE/SPIRE for workload identity, OAuth for token issuance) but applies them specifically to agents as first-class identity principals.
Tool Identity → NIST's Authorization Questions
NIST asks: "How do we establish least privilege for an agent when its actions aren't fully predictable?"Tool Identity solves this through explicit tool authorization:
- Each tool the agent can invoke is registered in a tool registry
- The agent's token includes a
tool_permissions[]claim listing authorized tools - Before each tool invocation, the MCP server verifies the tool is in the agent's permission set
This bounds what an agent can do, even if its reasoning leads it to want to do something else. Least privilege isn't about predicting every action — it's about constraining the action space.
Data Identity → NIST's Authorization Questions (Data Dimension)
NIST asks: "How do we determine sensitivity levels of data when aggregated by an agent?"Data Identity addresses this through classification-aware access:
- The agent's token includes a
data_classification_maxclaim (e.g., "internal", "confidential") - Before data access, the Data Identity checkpoint compares the data's classification against the token's ceiling
- Even if the underlying system would permit access, the token restricts it
This is critical for healthcare. An agent might have database permissions to read any record, but the Data Identity layer enforces that it can only access data matching its authorization level for this specific task.
Intent Identity → NIST's "Convey Intent" and Prompt Injection Questions
NIST asks: "How might an agent convey the intent of its actions? What controls minimize impact after prompt injection occurs?"This is where existing identity standards fall short. OAuth, OIDC, and SPIFFE don't have a concept of "intent." They verify who is acting, not why.
Intent Identity fills this gap:
- At token issuance, an
intent_hashis computed from the original request - This hash travels with the token through the entire execution flow
- At each iteration of the agent's execution loop, current actions are compared against the intent baseline
- Drift detection flags when the agent's behavior diverges from its original purpose
For prompt injection specifically: even if an injection successfully redirects the agent's reasoning, the Intent Identity layer detects that the actions no longer match the original intent. The injection might change what the agent wants to do, but it can't change the intent baseline that was captured before the agent encountered the malicious content.
Delegation Chain → NIST's "On Behalf Of" Questions
NIST asks: "How do we handle delegation of authority for 'on behalf of' scenarios?"The agentic token structure includes a delegation_chain[] claim:
- When Agent A delegates to Agent B, the chain grows:
["agent-a", "agent-b"] - Each agent in the chain can be verified for authorization
- The original
initiator_id(human or system) is preserved regardless of how many agents are involved
This maintains accountability even in complex multi-agent workflows. You can always trace back to who authorized the original action.
What NIST Gets Right
The NCCOE concept paper makes several important architectural choices:MCP as the protocol focus. They're not trying to invent a new agent communication protocol. They're taking MCP — which is gaining real adoption — and asking how to secure it. This is pragmatic.
Building on existing identity standards. OAuth 2.0/2.1, OIDC, SPIFFE/SPIRE, NGAC — these are mature, well-understood standards. Applying them to agents is easier than creating agent-specific identity systems from scratch.
Enterprise use cases first. They're explicitly scoping to enterprise deployments where you control the agents and the systems they access. External/untrusted agents are deferred to future work. This is the right sequencing.
Practice guide as the deliverable. Not just a framework document — a practice guide with implementation details from an NCCoE lab demonstration. This is what practitioners actually need.
What's Still Missing
Reading between the lines of NIST's questions, a few gaps emerge:Intent verification is acknowledged but not solved. NIST asks "how might an agent convey the intent of its actions?" but doesn't propose an answer. This is the hardest problem, and existing identity standards don't address it. The intent_hash approach in Multi-Layered AI Identity is one proposal, but the industry needs more work here.
Continuous verification vs. checkpoint verification. The concept paper focuses on authentication and authorization as discrete events. But agent execution is iterative — an agent might run hundreds of reasoning loops before completing a task. Verifying identity once at the start isn't sufficient. Intent Identity needs to be continuous, not checkpoint-based.
Healthcare isn't explicitly called out. The listening sessions will cover healthcare, finance, and education, but the concept paper's use cases are generic enterprise scenarios. Healthcare has specific requirements (HIPAA, data classification, clinical workflow integration) that need explicit attention.
What Healthcare Organizations Should Do
1. Submit comments. Both the CAISI RFI (March 9) and NCCOE concept paper (April 2) accept public input. If you're deploying agents in healthcare, your experience matters. NIST explicitly asks for "concrete examples, best practices, case studies, and actionable recommendations."2. Participate in listening sessions. CAISI will hold sector-specific listening sessions starting in April, with healthcare as one focus area. Watch the CAISI page for announcements.
3. Evaluate your current agent deployments. Use NIST's questions as an audit checklist:
- Can you identify which agents are running in your environment?
- How are agents authenticated? Do they have their own credentials or share service accounts?
- What authorizes an agent to invoke a specific tool or access specific data?
- Can you trace agent actions back to the human who initiated them?
- What happens if an agent encounters a prompt injection mid-execution?
4. Start building identity infrastructure for agents. Don't wait for final standards. The components NIST mentions — OAuth, SPIFFE/SPIRE, policy-based access control — are available now. The question is how to apply them to agents, and you can start experimenting.
5. Demand answers from vendors. If you're evaluating AI agent products for healthcare, ask:
- How does your agent authenticate to my systems?
- What identity claims does the agent token include?
- How do you enforce least privilege for tool access?
- How do you detect and respond to prompt injection?
- Can you provide audit trails that tie agent actions to originating human requests?
Vendors who can't answer these questions aren't ready for healthcare deployment.
The Bigger Picture
NIST launching this initiative signals that AI agent security is now a federal priority. The days of "move fast and figure out security later" are ending — at least for regulated industries.For healthcare specifically, this creates both pressure and opportunity:
- Pressure: Regulators will expect alignment with emerging NIST guidance. "We didn't know" won't be an acceptable answer.
- Opportunity: Organizations that build robust agent identity infrastructure now will be ahead when standards solidify.
The Multi-Layered AI Identity framework — Agent, Tool, Data, and Intent Identity — provides a structure for thinking through these challenges. NIST's questions validate that these are the right problems to solve. Now we need implementations.
